ὑπατείας τῶν δεσποτῶν ἡμῶν Κωνσταντίνου Σεβαστοῦ τὸ ϛ καὶ Κωνσταντίνου τοῦ ἐπιφανεστάτου Καίσαρος τὸ α. Αὐρηλίῳ Ἀδελφίῳ Ἀδελφίου ἀποδεδειγμένῳ πρυτάνει Ἑρμοῦ πόλεως Αὐρήλιος Προκλῖνος Ἀχιλλέως βουλευτοῦ τῆς αὐτῆς πόλεως. ὁμολογῶ ὀμνὺς τὸν θεῖον καὶ σε βάσμιον ὅρκον ἑκουσίως καὶ αὐθαιρέτως ἐγγεγυῆσθαι Αὐρήλιον Ὀλύμπιον Ἑρμοῦ βουλευτοῦ τῆς αὐτῆς πόλεως προχειρισθέντα ὑπὸ τῶν αὐτο τῆς βουλῆς εἰς ἐπιμέλειαν καὶ ἀνα κομιδὴν οἴνου ἀννώνης τῶν ἐπὶ Θηβαίδος διακειμένων γενναιοτάτων στρατιωτῶν ὃν καὶ παραστήσω πληρώσοντα τὴν ἐγχειρι σθεῖσαν αὐτῷ ἐπιμέλειανἐπιμέλιαν τοῦ οἴνου καὶ τῆς παραδόσεως ἐποίσονταἐποισομεν τὰ αὐτοῦ ἀπόχιμα α γράμματα. ἐὰν δὲ ἀφυστερήσῃ, ἐγὼ αὐτὸς τὸν ὑπὲρ αὐτοῦ λόγον ὑποστήσομαι, ἢ ἔνοχος εἴην τῷ θείῳ ὅρκῳ καὶ τῷ περὶ τούτου κινδύνῳ καὶ ἐπερωτηθεὶς ὡμολόγησα. ὑπατείας τῆς προκειμένης, Φαῶφι Αὐρήλιος Προκλῖνος Ἀχιλλέως ἐγγυῶμαι καὶ ὑποστήσομαι ὡς πρόκειται.
