Ἑρμοφίλῳ βασιλικῷ γραμματεῖ Ἀρσινοίτου Ἡρακλείδου μερίδος παρὰ Πτος Πετεεῦτος ἀπὸ κώμης Πτολεμαίδος Ὅρμου. ὁ συγγενής μου Σαραπίων Σαραπᾶ τοῦ τος μητρὸς Θερμούθεως τελῶν τὴν λαογραφίαν ἐπὶ τῆς προκειμένης κώμης Πτολεμαίδος Ὅρμου ἐτελεύ τησεν τῷ ὄντι μηνὶ Μεχεὶρ τοῦ ἐνεστῶτος κε ἔτους Μάρκου Αὐρηλίου Κομμόδου Ἀντωνίνου Σεβαστοῦ Εὐσεβοῦς. διὸ ἀξιῶ ταγῆναι αὐτοῦ τὸ ὄνομα ἐν τῇ τῶν τετελευτηκότων τάξει ὡς ἐπὶ τῶν ὁμοίων. κωμογραμματεῖ Πτολεμαίδος Ὅρμου. εἰ ταῖς ἀληθείαις ὁ προγεγραμμένους ἐτελεύτησεν, τὸ ἀκόλουθον ἐπιτέλει ὡς πρὸς σὲ τοῦ λόγου ὄντος ἐάν τι μὴ δεόντως γένηται.
