Ἀσκληπιάδῃ βασιλικῷ γραμματεῖ Ἀρσινοίτου Ἡρα κλείδου μερίδος παρὰ Πτολλᾶ τοῦ Πτολλᾶ ἀπὸ τῆς μητροπόλεως ἀναγραφομένου ἐπʼ ἀμφόδου Ἀπολλωνίου Παρεμβολῆς οἱ συνγενεῖς μου Διόσκορος Ἑρμᾶ τοῦ Πεθέως μητρὸς ύρεως καὶ Σπαρτᾶς Πο μητρὸς Ἀμμωνοῦτος καὶ Πτ Ἑρμᾶ τοῦ Πεθέως μητρὸς υρ εως οἱ γ λαογραφούμενοι ἐπὶ τοῦ αὐτοῦ ἀμφόδου Ἀπολλωνίου Πα ρεμβολῆς ἐτελεύτησαν τῷ μηνὶ τοῦ ἐνεστῶτος ιϛ ἔτους Αὐρηλίου Ἀντωνείνου Καίσαρος τοῦ κυρίου. διὸ ἀξιῶ ταγῆναι αὐτοὺς ἐν τῇ τῶν τετελευτηκότων τάξειτάξι ὡς ἐπὶ τῶν ὁμοίων γραμματεῦσι πόλεως. εἰ ταῖς ἀλη θείας οἱ προγεγραμμένοι ἐτελεύτησαν τὸ ἀκόλουθον ὡς ἐπὶ τῶν ὁ μοίων ἐπιτελεῖτε ὡς πρὸς ὑμᾶς τοῦ λόγου ἐσομένου ἐάν τι μὴ δε όντως πραχθῇ. σεσημείωμαι

An Asklê[piades, den Königlichen Schreiber des Hêra]kle[idês-Bezirks des Arsinoïtês], von Pt[ollas, Sohn des N.N., des Sohnes des] Ptollas, aus der Metr[opole] , registriert im Viertel des Lagers des Apollônios. Meine Verwandten Dios[koros], Sohn des Hermas, des Sohnes des Petheus, und der Mutter yris, und Spartas, Sohn des Po[[ und der] Ammô[n]ûs, und Pt Sohn des Hermas, des Sohnes des [Pe]theus, und der Mutter [[ yr]is, die 3 kopfsteuerpflichtig [im] selben Viertel des Lagers des Apollô[nios] sind gestorben im Monat des gegenwärtigen 16. Jahres des [Aurelius] Antonin[us] Caesar, des Herren. Hiermit bitte ich darum, sie [in das] Verzeichnis der Verstorbenen aufzunehmen wie in den ähnlichen Fällen. (2. Hand) An die Schreiber der Stadt . [Wenn] die oben genannten [tatsächlich] gestorben sind, dann führt das (daraus) Folgende wie in den ähnlichen Fällen aus, so daß das Risiko [bei Euch] liegt, wenn etwas Ungebührliches getan wird. (3. To Askle[piades, royal scribe of the Arsinoite nome, Hera]kle[ides division], from Pt[ollas, son of NN, the son] of Ptollas from the metr[opolis] , registered in the quarter of the camp of Apollonios. My relatives Dios[koros], son of Hermas, the son of Petheus, mother uris, and Spartas, son of Po[ , mother] Ammo[n]ous and Pt , son of Hermas, the son of [Pe]theus, mother [ ur]is, all 3 subject to poll-tax [in the] same quarter of the camp of Apollon[ios], died in the month of [NN] of the present year 16 of [Aurelius] Antonin[us] Caesar, the [lord]. Therefore I request that they be included in the list of those, who have died , as in similar cases. (m. 2) To the scribes of the city . [If the] above mentioned really died carry out what is appropriate as in similar cases with the risk [at your charge], if [anything contrary] to what is proper occurs. (m. 3) I have signed.
