Φλαυίοις Ἰσιδώρῳ καὶ Θεοδώρῳ ὀφφικιαλίοις τάξεως ἡγεμονίας Θηβαίδος παρὰ Αὐρηλίου Ἀπίωνος τῶν ἀρξάντων βουλευτῶν Πτολεμαίδος. ὁμολογῶ ὀμνὺς τὴν θείαν καὶ οὐράνιον τύχην τῶν τὰ πάντα νικώντωννικόντων δεσποτῶν ἡμῶν ΟὐαλεντινιανοῦΟὐαλεντιανοῦ καὶ Οὐάλεντος καὶ Γρατιανοῦ τῶν αἰωνίων Αὐγούστων ἐγγεγυῆσθαι Αὐρήλιον Σαραπίωνα Κάστορος ἀπὸ κεφαλαιωτῶν τρεισκαιδεκάτηςτρισκαιδεκάτης ἰνδικτίονος ἐπὶ τῷ αὐτὸν ἀπαντῆσαι εἰς τὴν τάξιν καὶ τὴν ἐμφάνειαν ἑαυτοῦ ποιήσασθαι, εἰ δὲ ἀπολειφθῇἀπολιφθῇ ἐγὼ αὐτὸς τὴν ὑπὲρ αὐτοῦ χώραν ἀποπληρώσω ἢ ἔνοχος ἔσομαι τῷ θείῳ ὅρκῳ καὶ τῷ περὶ τούτου ἐπηρτημένῳ κινδύνῳ καὶ ἐπερωτη θεὶς ὡμολόγησα. ὑπατείας Μοδέστου τοῦ λαμπροτάτου ἐπάρχου τοῦ ἱεροῦ πραιτωρίου καὶ Ἀρινθέου τοῦ λαμπροτάτου Μεσορὴ ιδ. Αὐρήλιος Ἀπίων ὁ προκείμενος ἐγγυῶμαι τὸν προκείμενον Σαραπίωνα ἐπὶ τῷ αὐτὸν ἀπαντῆσαι εἰς τὴν τάξιν ὡς πρόκειται. ἐγγύη Σαραπίωνος διὰ Ἀπίωνος Νικαντινόου κεφαλαιωτοῦ Πτολεμαΐδος.
