ρκη Οὐαλερίῳ Ἀμμωνιανῷ τῷ καὶ Γεροντίῳ λογιστῇ Ὀξυρυγχίτου παρὰ Αὐρηλίων Ἥρωνος καὶ Διδύμου Διοσκόρου ἀμφοτέρων ἀπὸ τῆς λαμπρᾶς καὶ λαμπροτάτης Ὀξυρυγχιτῶν πόλεως δημοσίων ἰατρῶν. ἐπεστάλημεν ὑπὸ σοῦ σήμερον ἥτις ἐστὶν Φαρμοῦθι ϛ ἐκ βιβλιδίων ἐπιδοθέντων σοι ὑπὸ Ἀπολλωνίου ὀφφικιαλίου ἡγουμένου Αἰγύπτου Ἡρκουλείας Αὐρηλίου Ἀντωνίου ὥστε γενέσθαι ἐπὶ τὴν οἰκίαν ἐν τῇ αὐτῇ πόλει καὶ τοῦτον ἐπιδεῖνἐφιδῖν καὶ ἣν ἂν καταλάβωμεν περὶ αὐτὸν διάθεσιν ἐγ γράφως προσφωνῆσαι. ὅθεν γενόμενοι ἔνθα ὁρῶ μεν αὐτὸν τοῦτον κλινήρηνκλεινήρην ὄντα πυρετίοιςπυραιτίοις ἀκρίτοιςαι συνεχόμενον· ὅπερ προσφωνοῦμεν. ὑπατείας Καικινίου Σαβίνου καὶ Οὐεττίου Ῥουφίνου τῶν λαμπροτάτων Φαρμοῦθι ϛ. Αὐρήλιος Ἥρων ἐπιδέδωκαἐπειδέδωκα προσφωνῶν ὡς πρόκειται. Αὐρήλιος Δίδυμος ἐπιδέδωκα προσφωνῶν ὡς πρόκειται.
