Φλαουίῳ Ἑρμίᾳ τῷ ἐλλογιμωτάτῳ σχολαστικῷ καὶ ἐκδίκῳ τῆς Ὀξυρυγχιτῶν πόλεως παρὰ Θεοφίλου ποιουμένου ηαικ ς εν τῷ φημα δὲ τ ταύτης τῆς λαμπρᾶς πόλεως οντι καὶ ταύτην τὴν ἐργασίαν ποιουμένῳ προσήκοντι τῷ κατὰ τῆς κεφαλῆς κόψας ἀφειδῶς καὶ θανατηφόρον ἐπενεγκὼν κατʼ αὐτοῦ πληγὴν ἔβαλεν ὡς εἶχεν ἐν τῷ πορθμίῳ, τῇ μὲν προθέσει τοῦτον ἀνελών, ὥσπερ ἡ παρʼ αὐτοῦ ἐπενεχθεῖσα μαρτυρήσει πληγή, τῇ δὲ τύχῃ τὸ ζῆν αὐτὸν κατα πράξας πως, ἐπὶ οὖν ἀμφίβολον ἔτι τὸ ζῆν ἔχει. παρακαλῶ τὴν σὴν ἐλλογιμότητα πρῶτόν γε πάντων ἐπιθεωρῆσαι Σουροῦς, εἶθʼ οὕτω κελεῦσαι τόν τε δημόσιον ταβουλάριον καὶ τοὺς ῥιπαρίους ταύτης τῆς λαμπρᾶς πόλεως καὶ αὐτοὺς αὐτοὺς ἐπιθεωρῆσαι τὸν σχεδὸν φονευθέντα Σουροῦς ὅπως τε ἔχει καὶ τὴν ἐπενεχθεῖσαν αὐτῷ πληγήν, καὶ ταῦτα γνώριμα καταστῆσαι τὴν ὑμετέραν ἐλλογιμότητα τῷ τὰ πάντα μεγαλοπρεπεστάτῳ καὶ σοφωτάτῳ κοινῷκυρίῳ ἡμῶν ἄρχοντι πρὸς τῶν γεγενημένων ἐκδίκησιν, κελεῦσαι δὲ καὶ τὸ περὶ τούτου ἐκσφράγισμα δοθῆναί μοι κατὰ τὸ ἔθος παρὰ τοῦ δημοσίου ταβουλαρίου, κύριέ μου λογιώτατε ἔκδικε.
